  Now You Can Fax Your Apps!    
  We are pleased to offer you the option to submit fully completed and legible applications by facsimile.  Only fax number 281.368.7344 may be used for submission of faxed Medicare Supplement applications.  This number is dedicated for fully faxed applications only.  Upon receipt, the Company will draft the initial premium, including the one time application fee.  The cover sheet, found at the link to the right, is required to be submitted with each faxed application.  Please be sure to notify the applicant that their account will be drafted upon receipt of the application.  Agent Webite User Guide
     
   Guaranteed Issue  
 
  • To avoid processing delays when submitting an application as "guaranteed issue" please include all documentation that qualifies the application as "guarantee issue" in accordance with state regulations.
  • For online applications, please fax or mail the documentation to the Underwriting department, referencing the application number.  This information is ALWAYS required.
  • Voluntary Disenrollment from a Medicare Advantage plan must be within the first 12 months of their first time enrolled in a Medicare Advantage plan.  It is not "guaranteed issue" if they have been enrolled longer than 12 months.  Only Plans A, B, C, F (including High Ded F) are available for guaranteed issue.
 
  Medicare Supplement Reminders  
 
  • Life Insurance is offered with all underwritten Medicare Supplement cases.
  • We will not accept an application more than ninety (90) days prior to the requested effective date, or the date the applicant becomes eligible for Medicare coverage.
  • Replacement forms are required for replacement of Medicare Supplement policies and Medicare Advantage plans.
  • A signed HIPAA Authorization is required for all Underwritten applications.
  • Be sure all required fields on the application are signed and initialed by the applicant.
  • Be sure to complete all required fields on the applications.
  • Be sure to include a signed agent page with the application. 
  • We do not backdate applications regardless of the application date.  The application must be received in the home office on or before the requested effective date.  The application may be faxed or submitted online to expedite receipt.
  • For online applications, please remind the applicant that their bank account will be drafted for their initial premium plus the application fee upon receipt of the application.  Enter the information carefully and verify it to avoid the draft being returned as "no account."
  • For online applications, do not enter a cell phone or alternate phone number in any billing or mailing address fields.  This results in incorrect mailing information.  Use the agent comments for this information.
